Primetime Fitness is a gym that allows you to engage in strenuous and high intensity exercise. You should consider whether this style of exercise is suitable to you. All members must complete the Pre-Workout Questionnaire which has been provided to you separately to this Membership Application.
Primetime Fitness is committed to training excellence and reserves the right to change operating hours, type and quantity of equipment, the type and frequency of classes and its fees, from time to time, at its sole discretion.
We don't have many restrictions, but we do take health and safety seriously. We ask all members to learn and abide by all the rules and polices set by Primetime Fitness and to acknowledge that these may change from time to time.
It is a condition of your membership that you understand and abide by the following Club Rules:

Respect to all gym users

Towels to be used on all machines including benches.

Weights to be replaced on weight racks after use.

Fit balls and stretch mats to be stored in correct location after use.

Water spillages either on the gym floor or in the bathroom are to be removed or notified to staff immediately.

Covered shoes to be worn at all times.
-
Equipment is to be shared if requested by another member.
-
Cleaning station is to be used to wipe over cardio equipment.

If you are unwell or have an infectious disease then we ask you to please be mindful of other gym users and refrain from attending the gym until you are well. If you are unsure then please consult a medical expert.

All personal belongings to be left in lockers or designated areas provided.

Under no circumstances are non-members to be let into the facility.

Equipment is to remain inside the facility at all times and is not to be used outside.

If equipment is faulty please attach a sign “OUT OF ORDER – DO NOT USE” in a clearly visible position.

Machines are NOT to be used when a “OUT OF ORDER – DO NOT USE” sign has been placed on the machine.

DO NOT allow non-members access to the gym. Doing so will incur additional fees and possible suspension.

Membership
All membership fees and schedules are subject to change. Our Membership Terms and Conditions have been provided to you separately to this Membership Application.
Dress & Conduct
All members should dress appropriately. This includes the wearing of a shirt and the use of a towel. No open toed shoes, sandals or thongs are allowed in the exercise area / training floor at any time.
Primetime Fitness is committed to the health, safety and welfare of each of its members and staff and will not tolerate unreasonable, threatening, obscene, harassing, indecent or illegal behavior. Primetime Fitness may, at its discretion, request members to leave the premises. Primetime Fitness, in addition to its common law rights and remedies, also reserves the right to terminate the membership of any member engaging in unacceptable behavior.
Members are liable for damages to our property that results from the willful or negligent conduct of a member (or of a minor for whom they are responsible).
Lost property or damage to a member’s property
Primetime Fitness is not responsible for lost or stolen property or damage to property. All property is stored at the member’s risk and we discourage members from bringing valuable articles to the Club.
Complaints Policy
Primetime Fitness welcomes all feedback from its members – positive or negative. We will try to resolve a complaint quickly and easily.
We invite you to discuss your concerns with our Club Manager straight away. We will respect your privacy at all times during this process and limit the discussions to only those concerned with your issue.
If you are not able to speak with our Club Manager, please email us your complaint to primetimefitnessmg@hotmail.com setting out the following information:

What happened – the date, was anyone else involved? Please provide a detailed description.

Do you want this fixed? If yes, do you have any ideas you want us to consider?

Can we talk to you further about this? If yes, please confirm your preferred method of contact – email or phone?
Once received, we will record the details of your complaint and, unless otherwise requested, we will send you a confirmation email or call you within one working day.
We will attempt to resolve all complaints as soon as possible but we ask for your patience during the process. We will keep you updated and contact you with details of how we believe we can fix things once we have investigated all options.
If you are not satisfied with our first proposal, please keep talking to us! We will move up the chain of command and find someone else who can help you. Once your issue has been resolved, we will close our records. Of course, if you remain unsatisfied, you are able to contact your state governing body but we will do all we can to resolve the issue first.
I, , acknowledge that I have voluntarily agreed to undertake is a dangerous activity and that by participating in in it I am exposed to certain risks.
The activity includes, but is not limited to, fitness classes, strength and conditioning training, resistance training, weight lifting, personal training sessions and other training services provided to the participant by Primetime Fitness.
I acknowledge and understand that whilst participating in such activity:

I may be injured – physically, emotionally, mentally, or I may die.

My personal property may be lost or damaged.

Other persons participating in such activity may cause me injury or may damage my property.
-
I may cause injury to other persons or damage their property.
-
The conditions in which the activity is conducted may vary without warning.

I may be injured or die or suffer damage to my property as a result of negligence or a breach of contract by Primetime Fitness.

There may be no or inadequate facilities for treatment or transport of me if I am injured.
-
I assume the risk and responsibility for any injury, death or property damage resulting from my participation in the activity.
I further acknowledge and understand that whilst participating in such activity:

I will follow the safety advice, training directions and exercise instructions of Primetime Fitness’ trainers, contractors, agents and volunteers.

If I feel unwell during training or feel pain and/or discomfort, I will cease participating and immediately advise a trainer.

If, for any reason, I require first aid or medical assistance, I authorise Primetime Fitness to seek emergency medical assistance on my behalf with the understanding that I am responsible for all costs incurred.
I acknowledge the release and indemnity contained within Primetime Fitness’ Membership Terms and Conditions, which has been provided separately to me.
I agree that:

I participate in the activity at my sole risk and responsibility.

I release, indemnify and hold harmless Primetime Fitness, its trustees, directors, owners, officers, employees, trainers, contractors, volunteers and agents, from and against all and any actions or claims which may be made by me, or on my behalf by other parties, arising out of any injury, loss, damage or death caused to me or my personal property whether by negligence, breach of contract or in any other way whatsoever.
I also agree that in the event I am injured or my personal property is damaged, I will bring no claim, legal or otherwise against Primetime Fitness in respect of that injury or damage. This agreement should be effective and binding upon me, my successors, representatives, heirs, executors, assigns or transferees. If any portion of this agreement is held invalid, I agree that the remainder of the agreement shall remain in full legal force and effect.
